Carr Gomm’s Coming Home Project – helping people identify what’s important to them in a home.
Carr Gomm is working with colleagues from across the Argyll and Bute Health and Social Care Partnership (A&B HSCP), including voluntary and independent sector housing and specialist care providers and with individuals and families to review, plan and inform the development of housing and support solutions for individuals with a range of complex care, support and protection requirements. This work is set in the context of the relevant national and local strategies, including but not limited to:

As part of Carr Gomm’s collaboration with A&B HSCP, the Coming Home Project (CHP) team is conducting independent reviews for individuals originally from the community, currently living in out-of-area placements or long hospital admissions. The purpose of these independent reviews is not only to assess current support arrangements but also to identify opportunities for enhancing quality of life. Key areas of focus will include, but are not limited to, preferred living arrangements, meaningful relationships and personal goals. We want to know what home looks like and feels like for the individual and what’s important to them so that they can be supported to live their best life.
We have also been working in partnership with current support & housing providers to gain a better overview of current service provision within Argyll and Bute. The CHP team were invited to attend the A&B Adult day services, all localities event, “It’s a Knockout” in August 24. This was a great opportunity to celebrate these amazing services available for A&B residents and experience their person-centred philosophy & accomplishment framework values first-hand. It also gave the CHP team a chance to interact & speak with individuals about their experiences of support. This event was not only a lot of fun to be part of but helped the CHP team understand the achievements, challenges A&B Adult day services have and their aspirations for the future.
